Sildenafil Basics is a potent, selective cyclic guanosine monophosphate (cGMP) specific inhibitor of phosphodiesterase type 5 (PDE5).
The physiological mechanism of erection is based on the release of nitric oxide (NO) in the erectile tissue during sexual stimulation. This, in turn, leads to an increase in cGMP levels, resulting in relaxation of smooth muscle tissue in the cavernous bodies and an increase in blood flow in the cavernous bodies of the penis.
Sildenafil Basics 100mg/ 50 mg does not have a direct relaxing effect on the corpus cavernosum but enhances the relaxing effect of nitric oxide by inhibiting PDE 5, which is responsible for breaking down cGMP in the corpus cavernosum.
In vitro studies have shown that sildenafil acts selectively against PDE5. Its activity compared to other known phosphodiesterases is much lower.
A prerequisite for the effectiveness of Sildenafil Basics 50 mg/ 100mg is sexual stimulation.
The use of Sildenafil Basics in doses up to 100 mg resulted in a slight, transient decrease in blood pressure. The antihypertensive effect is due to the vasodilating effect of Sildenafil Basics, which is accompanied by an increase in the level of cGMP in vascular smooth muscle cells.
A mild and transient impairment of color perception (blue/green) was noted in some patients 1 hour after administration of Sildenafil Basics 100mg. Color perception was restored after two hours. Color vision impairment is caused by inhibition of PDE 6, which is involved in the transmission of light in the retina. Sildenafil has no effect on visual acuity, contrast perception, electroretinogram values, intraocular pressure or pupil diameter.

To minimize the risk of postural hypotension in patients taking alpha-blockers, Sildenafil Basics should be initiated only after haemodynamic stabilization has been achieved in these patients. Reducing the starting dose of sildenafil should also be considered.
